Innovative Chronic Illness Care

Patients throughout the country have joined with their care providers to participate in an innovative program that is using a new approach to treat diabetes and asthma. This program has already improved both the quality of care these patients receive and, more importantly, their health.

Commonly known as IPIP, the Improving Performance in Practice initiative creates a system to assist primary care physicians and their practice teams to assess and improve the quality of chronic illness and preventive care. IPIP is a learning network that allows practices to track their progress and work together to achieve the best results and to share the benefits of their improvement and experience with others.

It sounds simple – and it is. But the problem is that until now, it was very difficult to share information and provide benchmarks to improve everyone’s care.

Now, thanks to technology advances and this innovative program, it is possible for primary care practices to cooperate to achieve widespread improvement. Diabetes and asthma are common, long term conditions that can be well managed. IPIP offers proven steps practices and patients can do to significantly improve care and outcomes for patients. IPIP empowers everyone in the process - patients, families and health care providers – to work together to provide preventive services and treat chronic illness while taking advantage of the latest and best medical knowledge.
