Requirements

Requirements for states participating in IPIP

 

  • Organize an IPIP Steering Committee, including a broad constituency of stakeholders from within the state
  • Identify a physician champion who will provide leadership for the IPIP effort
  • Name an IPIP state director to lead implementation
  • Identify and/or create networks of primary care practices from different regions
  • Be prepared to recruit at least 30 primary care practices in the first year
  • Assist practices to identify electronic solutions, such as registries
  • Commit to the monthly submission (by practices) of IPIP measures
  • Review monthly aggregate reports of performance data and provide analyses of performance, including areas for improvement and strategies to achieve outcomes
  • Develop state and/or regional Learning Collaboratives, name leaders and identify faculty; engage practices and focus on outcomes
  • Incorporate the RWJF Aligning Forces for Quality sites into the IPIP program
  • Align with other quality improvement initiatives in the state, including those sponsored by state government
  • Develop strategies for long-term sustainability

 

Requirements for Practices Participating in IPIP

 

  • Primary care practice: family medicine, pediatrics or internal medicine
  • Practice in an IPIP state: CO, MI, MN, NC, PA, WA or WI
  • Choose diabetes or asthma as a focus of improvement
  • Identify a physician leader in the practice
  • Have a patient registry to manage the population, or, be able to extract process and outcome data from an existing electronic medical record
  • Provide data every month on IPIP measures, using specifications for numerator and denominator selection
  • Assign roles to each member of the practice team
  • Use a planned care template to manage care
  • Use protocols to ensure adherence to evidence-based guidelines
  • Provide patient self-management education and support
  • Engage in small tests of change (PDSAs) to improve care
