Learning Collaboratives
IPIP Learning Collaboratives
IPIP uses the Learning Collaborative model in several of its states and regions. The IPIP Collaborative model brings practice teams together to engage in collective learning about methods to improve care for patients with diabetes and asthma. Practices share data on their own performance and communicate best practices in order to accelerate improvement for all participants. Key features include:
- Each practice brings a small team (3-4 people) to each learning session
- The team includes a physician champion, clinical team leader, and administrative leader (such as the practice manager)
- The team commits to attending periodic learning sessions (at least 3 over a 12-18 month period)
- Pre-work (such as data collection and reading) and activity between sessions are requirements of participation
- Teams use their own performance data to monitor progress and create improvement in outcomes
- Improvement is achieved through redesign of office practice elements using the PDSA model for improvement
